Middle ear problem : You should see your primary care physician to examine your ears. You may have fluid (effusion) in the middle ear (on the other side of the eardrum) which could be due to allergy or infection. The ringing may also be due to an inner ear problem but that shouldn't cause pain if your head is under water. You night need to see an ear, nose, and throat (ENT) specialist.
