Infection/crack: Any infection /skin crack may cause this type of pain.Get checked by your pcp.

Yeast still likely: Even if you have no sign, nor the baby any sign, that history sounds suspicous for yeast. Get a good assessment by an experienced lactation consultant, and try to find a sympathetic doctor who would be willing to listen to the treatment suggestions by the lactation consultant. A provider that is an lc and an arnp would be able to see you and probably write a script for treationg you and the baby.
