Depends: The problem could be in the neck, the structures between the neck and shoulder or the shoulder itself. If the discomfort is worse turning your head to the right, it suggests muscle type problem in the neck rather than a pinched nerve coming out of the neck. If shoulder motion is very uncomfortable or limited it suggests problems(?Bursitis) there. The jaw discomfort may be unrelated. See your md.

Surely you jest: Can't make a Dx without an examination/imaging/appropriate labs, to determine cause of the stiffness and thus appropriate treatment. Start with your GP. If it hasn't gone away in months your body is crying for professional help.
