My take for you ...: For a 31-yr man still troubled with tight foreskin, the most reasonable bid for long-term care is circumcision, which could be done "easily" under informed consent and aseptic condition as an outpatient. Meanwhile, don't forget to pull foreskin down after cleaning and drying so to prevent its hangup - parphimosis. So, seek timely professional evaluation and counseling for such.
