Yes: Can develop paraphimosis if foreskin not returned over glans. Worth trying prescription strength steroid cream, like betamethasone, or a minor prepucioplasty if you wish to retain foreskin. Otherwise definitive treatment would be a circumcision.

My take for you ...: For a 31-yr man still troubled with tight foreskin, the most reasonable bid for long-term care is circumcision, which could be done "easily" under informed consent and aseptic condition as an outpatient. Meanwhile, don't forget to pull foreskin down after cleaning and drying so to prevent its hangup - parphimosis. So, seek timely professional evaluation and counseling for such.
