Usually not: Currently majority of breast biopsies are (should be) done percutaneously by a needle. This is done usually by ultrasound guidance or x-ray guidance so called stereotactic biopsy. If the tumor is large and easily palpable it can also be done with palpation guidance. The doctor who is doing the procedure uses local anesthetic and most of the time the procedure is done with very little pain.
