How good is therapy: Any of these could contribute. Often sleep apnea needs to be re-evaluated to be certain your sleep is restful and that there is no additional sleep disorder. Is the RA well controlled. Is the diabetes controlled? Your physicians and your rheumatologistt will go through these various problems and assess them.

Talk to your DR: Amlodipine ; besylate are the same thing. The higher the dose the more you will feel sleepy. You may be taking too much. No reason to take both. Diovan (valsartan) can also make you sleepy. You should be on a daily uncoated baby aspirin. The longer you take metformin, the lower your b12. Your dr can check it. The more sedentary you are the more you will feel tired. You need to be moving. Sleep deprivation can.

Weight?: It would be interesting to find out what is your current weight, and consider adjusting it to your optimal bmi. Also (if indicated/monitored by your dr) other lifestyle changes should be explored, like exercise in a systematic manner. There are things that most people can not do anymore at a certain point in life, but good news: new good things will possibly make you feel better. Good luck !
