Rectal prolapse: It sounds like your brother has a rectal prolapse. In children the rectum can be pushed back into place. But it will be necessary to treat your brother's constipation with diet and medication.

Prolapse: What you are describing soiunds like rectal proplapse. This is when the rectal lining everts with excess pressure as in constipation. Treat the constipation and it should resolve. If it persists, he may need a seat chloride test to rule out a genetic cause, but this would be rare. cystid fibrosis should be ruled out if prolapse persists. this can be done with sweat chloride test.
