No: A crl of 4.6mm is approximately 6 weeks. If there is a normal fetal heartbeat, i wouldn't be concerned about the discrepancy in gestational sac size. Sometimes gestational sacs are elliptical in shape and will give a false size unless it is measured in 3 planes and a mean sac diameter is used. Also large gestational sacs are not associated with non-viability with a normal heartbeat.
