Need Orthopedist: Trimalleolar fracture is fx of medial and lateral bone sticking out at both side of ankle and piece of calcaneus (heel) bone. Once the bones completely heal, one can elect to leave the hardware in for years, or remove it. If one has an experienced orthopedist that set the fractures, or hardware causing pain, remove it. Can get second opinion, but that ortho will likely refer back to do it.

Little: If your injury has healed fully, there aren't any cons, besides the risk of undergoing surgery itself and its related anesthesia. Generally, most ankle fixation hardware can be left in place indefinitely, except for a special screw called a transsyndesmotic screw or if fixation has become painful. In your case, it is highly likely that you have this and it is necessary remove it.
