Missed: Because you missed the spot where the cavity formed. You should be brushing a full 2 minutes by the clock, especially in the danger zone between the wires and you gum line. Do you water pik? Do you use dental disclosing tablets? Do you see your dentist regularly for maintenance? Ask your orthodontist why you have a cavity.

Details: You got a cavity because you probably did not do a full job , flossing , then brushing taking your time to get to hard to reach areas like under the wire or between brackets. The use of electric tooth brush or a water pick might help greatly , good luck.
