Decreased oxygen lev: This is a clinical decision since patients with symptoms of hypoxemia (low oxygen) most likely will not improve until their oxygen carrying capacity (red blood cells and hemoglobin) are increased to support the normal function of the body.
In cases of acute bleeding of 2 or more liters of blood, the patient most likely will need a transfusion (our body has 4-6 liters of blood or 10 pints).

Several possibly: Reactions to blood transfusions are very rare now with the techniques that we have to "crossmatch" blood. The most common reaction is have a rise in temperature. Next would be an allergic reaction. Most of these are not severe. The worst reaction is called a hemolytic reaction. These occur when there are important incompatibilities between your blood type and the type you have been given.

Filter: The blood in the plastic bag is actually a living thing so it is natural that some of the red blood cells deteriorate over time. The filter assures us that the dead red blood cells get filtered out and only the good, healthy cells get into your blood stream.

Also filters help prevent clots to be insfused during the transfusion. Some special filters can trap the white blood cells to prevent some febrile reactions related to their contents.

Varies: Worst case of incompatible blood is major txfsn reaction w/fever back pain red urine jaundice shock and kidney failure. Minor incompatibility can cause delayed reaction with drop in blood count about 7 days later w/o other symptoms. Fever only reactions are common and harmless. Allergic reactions with hives, sometimes low BP can occur. Only the major reaction is checked for and shouldn't happen
