Yes - go to the ER : Or an acute care clinic. You may need wound care to include debridment, tetanus shot ; possibly antibiotics. Take this one seriously. Be well.

Timing is important.: If redness resulted immediately following the puncture wound it could be a result of histamine release and not infection. If redness, pain, swelling developed days later then that could be consistent with infection. If the latter occurred you should seek medical attention.
