Please explain why would clots keep forming after a thrombectomy?

Where are clots?: Either 1) disease in the blood vessel, 2) a clotting disorder or 3) clot in the heart or other location which is traveling and lodging at site of thrombectomy (embolus).

Just how risky is I & D of large hematoma over the brachial artery? Had brachial artery thrombectomy over 4 weeks ago and it is not going down & has caused incision dihiscense. Surgery is trying home cares first (maybe d/t COVID?) & it doesn't help.
