Exam: Unilateral shoulder pain in a forties male is not easily diagnosed. Shoulder is a very complicated biomechanical structure. The most valuable information is gained by a doctor, such as an osteopath, who performs a hands-on evaluation while the shoulder is functioning. Absent acute mechanical injury, imaging studies provide little information. Once surgery has been performed all bets are off.
