Ask for an estimate: It depends on your insurance status - if you have insurance you can work with them to estimate YOUR cost of the surgery. If you don't then ask your doctor's business manager for a proposal and estimate. Since you would pay everything yourself you can also to a certain extent negotiate at that point.
