They don't always: Gallstone pain can be in the right upper quadrant of the abdomen, the upper central portion of the abdomen, the right upper back and shoulder blade area, the right shoulder, etc. Other symptoms include bloating, belching, fatty food intolerance, nausea, vomiting. With complications symptoms can get severe including jaundice, fever/chills, pancreatitis, etc.
