High unless treated: Leishmaniasis, even cutaneous, if left untreated will progress and cause complications. There are cases when cutaneous leishmaniasis stalls in its progression due to strong immune response from the host, but i would not count on luck in this case and seek treatment immediately. Visceral leishmaniasis carries a grim prognosis unless treated.

Complications: Untreated Visceral Lleishmaniasis is usually fatal. The people who are likely to have complications are those who are severely malnourished, those with poor immune systems and often people who are too poor to pay for treatment (or who come from third world areas where proper care isn't available)
