Is a broken ankle very painful? Or can it be only mildly painful and fool you into thinking it was a sprain?

Both: Actually, both a sprain (ligament tear) and a broken ankle (fracture) are quite painful. Some would rate severe sprains as more painful, so it is difficult to quantitate. Every person has different responses to pain stimuli, so one person's severe pain is another person's bee sting. So, there are definitely cases where broken ankles are missed.
