Both: Actually, both a sprain (ligament tear) and a broken ankle (fracture) are quite painful. Some would rate severe sprains as more painful, so it is difficult to quantitate. Every person has different responses to pain stimuli, so one person's severe pain is another person's bee sting. So, there are definitely cases where broken ankles are missed.

It can fool you: Amont of pain can't be used to distinquish thextwo or amount of swelling xray only reliable way of differentiating.
