No: If there's a little cancer in your pilonidal cyst (and it can happen, though unlikely, if things have been bad there for a while), the pathologist will find it.

Not typically. : But in very rare cases a chronic inflammation (like from a pilonidal cyst) can result in skin damage which could turn into cancer.
