Humans are human...: Sometimes things just happen. Other times people make mistakes. Some possibilities: tubes were not filled to the required amount, blood unintentionally clotted in the tubes, tubes got lost, wrong tubes were used, tubes were not processed within the required time frame, tubes were kept at the wrong temperature, etc... The lab staff usually takes the blood samples again at no additional charge.
