Comfort care: Metastatic lung cancer is difficult to deal with, both as patient and family. The transfusion and elevated heart rate are signs of blood loss and hypotension. Treatment should be focused on making the remaining time as comfortable as possible.

HR 140 is concerning: Heart rate of 140 may be acute atrial fibrillation or worse. She should see a doctor immediately. There are multiple medications available to lower the heart rate.
