Possible stroke: Those are worrisome signs, and i recommend that you have her evaluated right away at your local er. Time is of the essence here!

Be seen: I would certainly be seen immediately. Elderly can go into delirium with relatively simple things like a urinary tract infection, in the other hand she might have had a major stroke.
