Focused evaluation: I'm not sure what kind of evaluations you've had about this problem -- and it's not clear what contributes to "i can't [get involved]." a 15-minute medication management-oriented appointment isn't likely to allow opportunity to explore whatever is limiting you. You might ask for in-depth psychological evaluation and testing. This could help flesh things out so effective help can be given.

"Why" may not be as: Important as how to change it. If you have not found psychiatrists helpful, try another mental health professional. Once you find someone with whom you are comfortable and in whom you have confidence, you should be able to make progress.

Psychological tests: It sounds like they have exhausted medically treatable conditions. This is where psychological testing can help a lot. It will help a psychologist and your psychiatrist have a better understanding of who you are and the inner workings of your mind. Some people have avoidant or schizoid personalities. Knowing if you have either is important in treatment planning for someone like yourself.

Neuropsych Assessment: I would suggest consulting with a clinical neuropsychologist about getting a neuropsychological assessment. The neuropsychologist should be able to better to recommend including certain types of tests based on background information you provide.
