Probably: Obstruction or varicose vein (the latter is uncommon on the upper extremities however), see a vascular surgeon.

Dilated veins: Veins normally dilate when the arms are down. If the veins remain dilated with the arm held up at shoulder level it would suggest an obstruction from clot or scar tissue perhaps.
