My 20 month baby has just a few words - 15, 20 words (most of them just the 1st sylabel) but understand everything. should i be worried ?

Probably not: 15-20 words, using 2-3 word phrases, pointing to a few body parts when asked, knowing what a toy is such as a ball or a car, understanding simple directions are generally what is expected at this age, but if you are really worried i'd suggest you discuss it with your pediatrician at your child's 2 year checkup.
