Very unlikely: Back pain is an uncommon presentation of cancer, and a common presenting symptom of many benign conditions, some of which are not cancerous but are still serious. If it is a new pain that you have never had before, and is severe, you should definitely see a doctor. It could be an unusual presentation of heart disease... Or just a pulled muscle.

Acute thoracic spine: Acute thoracic spine pain is serious problem. Causes can be compression vertebral fx, herniated disc, slippage of vertebra, spondylolysis, bone cancer , aortic dissection! cancer diagnosis would be made by biopsy and mri. I think you should go to er for history, exam, MRI ; related tests. Thanks.
