Paying bills: I am sure no one will refuse cash as payment and there is no reason why you can't request that no bill be sent to your house and have that request honored. Just make sure it's clear to the receptionist or financial department.

Don't see why not: Discuss your wishes with the office staff at your visit. There is no need to send a bill if there isn't a balance, right?
