i've been on antibiotics 16 times this year but the doctor won't refer my to a consultant or specialist, what do you think the problem could be?

16???: You mentioned only kidney infection and tonsillitis without giving other details about the reasons for the antibiotics. 16 courses is a lot (i haven't taken that many in my lifetime!). Kidney infection is a good reason for antibiotics, but in general tonsillitis is not. Without knowing more details, i can't tell whether you need a specialist, or just a new doctor.
