Much has to go right: There could be something wrong with the placenta or the baby or both. Medication taken at the wrong time could be disruptive, implantation may not be in the best spot, infections occur, rarely trauma. It is nearly always something completely out of your control, and the body is very efficient at cutting short a pregnancy that just isn't going right.
