"Worst headache": Rupture and bleeding with severe headache. Other side- effects, along with bleeding risk, include possible milder headaches, and/or less commonly symptoms of mass effect from some larger and giant aneurysms. Mass effect symptoms can include double vision or even weakness on one side if the body in extreme cases. Lastly, and even less commonly, some partially clotted aneurysms can cause strokes.

Depends: Very small and unruptured aneurysm may not cause any symptoms. Sometimes the growth can come with headaches. In specific locations or with greater size aneurysm can have mass effect and compress nerves or brain structures. In the case of a rupture the usual presentation is the worst headache of the patients life and comes with a whole set of problems.
