Minimal: CAD does not develop overnight. Plaques develop early and grow over time. Family history contributes, but hypertension, diabetes, high cholesterol, obesity, and other factors are involved. Smoking is a large part of this, since it contributes to making the blockages symptomatic. If you are concerned, then smoking is an incredibly stupid thing to do.
