Probably not: Angina doesn't occur in a localized "spot" and typically patients will press their fist against their mid sternum and report a sensation of heaviness or pressure "like someone pressing down" on them. The words twinge and spot suggest a muscular cause. Depending on your specifics, though, it's never a bad idea to discuss this with your doctor and get a complete evaluation.

Possibly: Probably worth having a stress test to better evaluate it especially if you also have high blood pressure/cholesterol, diabetes, or smoke.
