This is a: lifestyle issue. All of us change our eating and/or exercising habits every now and then, but not all of us stick to it. Surround yourself with groups of people who think and act as you wish and then challenge your history whenever you feel resistance. A therapist might help, as might a gym coach, dietician, etc. You're probably not lazy, just the victim of poor training.
