Same as now: Some lived a normal life and some got infections and died. The proportion is just different. Antibiotics and vaccines have both resulted in a better life for everyone.

Lots more died: Forgive my frankness. Go to an old cemetery and see how many gravestones are those of young children. This results primarily from antibiotics and immunizations. Anybody who thinks that "natural healing is better" should make the trip with you. Thanks for asking, and best wishes.
