A member asked:

For medium-active crohn's disease, is remicade (infliximab) better off as the first line of treatment than methotrexate? azoran has been ruled out.

A doctor has provided 1 answer

See details: Often the combination of both is the most effective therapy. Often Methotrexate is tried first because it is easier to administer, is much less expensive and can be effective as a single agent. However, for severe disease i would opt for Remicade (infliximab) either alone or in combination as the first choice.
