Not cancer: Cancer of the testis announces itself as a mass. I'm going to assume that you do not have a palpable mass and that the painful testis is not noticeably larger than the other. Most testicular pain that varies with position is from minor trauma, likely unnoticed, that will probably heal up when you start wearing a more effective supporter for yourself.
