2 year old fever of 103 and on antibiotics for 4 days uti and uri.noticing white patches on throat now. not eating little fluid intake breath bad.

So much going on...: There is so much going on your child needs reexamination. A 2-year old with 4 days of 103 fever while already on antibiotics requires urgent reassessment. It is my concern at best that the oral antibiotic may require changing; at worst, your toddler may require IV fluids and IV antibiotics--if the UTI is actually pyelonephritis, a serious kidney infection. White patches may be thrush or stomatitis.
