Cold extremities : For 23 year old male with constant cold extremities, the poor circulation is certainly a concern. Please see your doctor and get blood tests for thyroid, anemia and some connective tissue disease like lupus. Also your doctor has to examine for raynaud's or buerger's disease. Stop smoking. For time being you should wear the double layers of clothes. Stay in warm environment at work and home.

Cold fingers: probably not your circulation but more probable to be raynauds see your md for evaluation to determine of you have thyroid disorder another possibility could be nerve compression from neck do you do weight lifting?
