Need a good eval: If this is involving your thumb it is less likely a median nerve issue. Involving your upper arm as opens the door for problems that might originate in the spine or spinal cord. Evaluation should include a nerve conduction test and examination try to determine the level of involvement. If your current neurologist is not helping at this time to change. Your pcp will help you find a good one.

Nerve test: If i interpret your question correctly, there seems to some type of neuropathy in arm. Carpal tunnel is one of the most common ones. The median nerve is either inflamed or compressed on the palm side of the wrist. We treat this with wrist splinting, anti-inflammatories, injections & surgery for more severe cases. A nerve conduction study can help confirm the cause of your symptoms.
