Plastic Surgery: Most surgeons do not charge for the initial visit when one is considering surgeries that are self pay or 'vanity' surgeries. You can ask about this when you make an appointment at the plastic surgeon you choose.

Usually: Most plastic surgeons charge a fee (usually in the $50-100 range) for an evaluation. Most will credit that towards any surgical procedures resulting from that consultation. Even if it steers you away from the surgeon, it's money well spent.

Depends on the DR: Some surgeons charge for a consultation, others do not. Clear that with the office when you call..
