My baby is gaining weight but dropping percentiles. 75th percentile at birth and is now 10th-25th at 4 months. fully breastfed. does he need formula?

Weight drop : From lack of calories. If that is all you can produce , by all means , supplement with formula. Your baby will be happier, satisfied, not looking for food every hour , and sleeps through the night and be up on the chart again!

Dropping percentile: His weight gain may not be adequate. He should be gaining about 800-900 grams/month (around 1.5 to 2 lbs). If he's dropping his percentiles, then he's not gaining enough. Breastfeeding is a great way to feed your baby, but some women need to be coached how to do it correctly. All of our children were breastfed exclusively and did well. See lactation specialist.
