No: Usually incisional hernias are repaired laparoscopically by placing mesh to cover the defect from within the abdomen. In an open case, the mesh can be implanted above, below, or in between the abdominal wall.

Sometimes but: ... ideally hernia repairs avoid tension. the more tension the more likely the hernia recurs
