LLAbd pain: 12 year is a very long time. You did not mention about diagnostic tests. Please have detailed history, thorough physical exam, including pelvic. You also need us of abdomen ; pelvis. Colonoscopy and MRI of lumbosacral spine to find out the cause. Sometimes surgical scar tissue cause pain but not radiation to leg. With menstrual, ovaries evaluation needed. See your gyn. Thanks.
