Alzheimer's dementia: Alzheimer's is a disease process that lasts generally about 10 years. It is not that someone starts out with dementia and then progresses to alzheimer's, rather, they start out with alzheimer's and remain with alzheimer's until their death. The diagnosis of alzheimer's is a clinical diagnosis based on typical mental status examination findings. Majority of such patients are diagnosed by family practitioners and neurologists. About 10% are diagnosed by psychiatrists. Best wishes.

Dementia: Severe dementia is also progressive whether it is alzheimer's type or not. Unfortunately, we can't really test for alz from a medical, neurological, or neuropsychological perspective.
