Self Esteem: You were not born with low self esteem, it is a view of oneself you learned through interactions with others. You need truthful feedback from rational objective people to re-write the inaccurate story you wrote early in life. My book "changing your addresses this issue, also consider group therapy. Best.

Change It!: If you would like to try the self help route - i found "the feeling good book" by david burns to be very helpful. You not to not only read it but also practice the concepts. Another good option would be working with a therapist, what is important to know is that you do have options. Take care.

Seek help : Make an apt with your therapist that accepts your insurance or call around to find someone who is willing to work with you on your fee basis. See a psychiatrist for possible need for medicine for depression or anxiety or both. Do not wait, this gets worse.
