Ears: Fluid behind the eardrum that causes pain is common. It doesn't always mean bacterial infection. Even when there is bacterial infection, it doesn't always need antibiotic and the wait and see approach is perfectly appropriate. In these situations i recommend ibuprofin (or acetaminophen) for pain. But if the pain persists for more than another 2 days, your son should be rechecked.

Middle ear fluid: Middle ear fluid is a common incidental finding and can be associated with a muffled like feeling in kids (like not being able to pop your ears after flying).The fluid itself is benign and may persist for weeks at a time without problems or might become infected by throat germs over less than 8 hours.Ibuprofen is usually adequate for pain with recheck if or when the kid is running fever.

Manage the pain: Most fluid behind the ear goes away on its own. The best way to manage this is using over the counter pain medications such as Acetaminophen and ibuprofen, assuming he has no allergies to these medications. Be sure to follow the directions on the package. If the pain worsens over the next day or two, you should have his ear looked at again.
