Most comfortable: All neurosurgeons should be trained in managing chiari malformations after completing residency. (i am assuming chiari i.) some just do more cases than others, but anyone who does cranial surgery should feel comfortable with it. I would say it is a pretty run-of-the-mill and straightforward problem in general.
