very severe: When BP exceeds 200/120, it approaches a level considered to be a hypertensive urgency or emergency. The symptoms can be angina (chest discomfort), heart failure (shortness of breath), or encephalopathy (confusion and sometimes headache). But understand: headache is common and is infrequently associated with high blood pressure.
