Distressed?: I am assuming your son is fairly young. If he is showing any signs of distress when breathing like using lots of his chest muscles, belly muscles, flaring his nostrils, breathing fast, then yes he needs to go to the er tonight. It sounds like you've tried the therapy at home you can (his inhaler) and if not getting better, he needs more care and monitoring.

Yes: If you suspect your young child has pneumonia with chills, fever and cough you should immediately take him to your er for diagnosis and treatment.
