Diet, exercise and: Weight loss are the only "best" med. Metformin is generally not stopped unless there's a specific reason. You add medication instead of substituting. Invokana is a good drug, works better with metformin. Ask your doctor, as several different meds can be added in, including glp-1 agonist therapy, or Insulin if necessary.

Depends: Diabetes has multiple causes. Thus rational pharmacology should adress as many causes as possible. Insulin may be needed if you are not able to control your blood glucose levels with oral agents or incretins.
